IYSA affiliated teams (IWSL, YSSL, CIYSL, SISL)

a. A copy of IYSA Tournament Roster form 

b. A copy of League roster (IWSL, YSSL, etc)

c. An approved Guest Player Form if you are using guest players.

Note: Players from the same club are not considered guest players and only need to provide a copy of each teams’ League roster.

d. IYSA Youth Program Background Check and Concussion Awareness Certification Form.

e. Emergency contact information for team manager and coach (cell phone)

Note: you do not need to submit IYSA Medical Release forms; however you are required to have them at ALL games. Any player receiving medical treatment will need to provide the release.
